How to provide your prescription to Patient Choice

We can receive your prescriptions in four ways.

  • You can ask your GP or clinic to send them to us. They’ll know what to do.
  • You can call our freephone order line and speak with us - 0800 862 0686.
  • You can send them to us online using our web form.
  • Or you can post them to ‘Freepost Patient Choice’. That’s all the postman needs to know!
